IMPORTANT INFORMATION FOR MINORS
Required Documentation:
The NEW Minor Waiver Form MUST be signed by BOTH Birth Parents & notarized prior to arrival!
Legal Documentation Required:
LEGAL proof is REQUIRED if there is only one parent and/or guardian or if the parents' last name/s are different from the Minors (i.e.: birth cert, adoption paperwork, death certificate, sole custody paperwork).
Appointing Guardianship:
If the legal guardian and/or birth parent/s are unable to bring the Minor to the track themselves, they can appoint guardianship to a person over 18yrs old. The Guardianship Form needs to be completed & notarized in advance.
